Challenge Entry Form
Register between January 1st 2011 and June 30th 2011.
To register your team or workplace for the challenge is really very easy. All you have to do is download the .pdf entry form below, save it to your documents folder and complete it in one of two ways;
The first way is to open the document and use the 'Typewriter' facilty which is located at the top of the document; simply place the cursor key on the typewriter icon to activate it, then simply type the relevant information anywhere on the document (remember to save any changes!). Then once complete email it as an attachment to; lesley@firefithealthchallenge.co.uk.
Or, download the form, print it off and complete the form by hand before then posting it to;
